​​Asthma is a chronic disease of the airways that makes breathing difficult.  There is swelling of the airways that can cause coughing, wheezing, shortness of breath, and chest tightness.  With proper asthma treatment, you can live well with this condition. Poorly controlled asthma can lead to multiple visits to the emergency room and even hospital admission.

IMCare offers asthma disease management to members five years and older through our Ways to Wellness program. This educational program can teach you how to manage your asthma.  Ways to Wellness member education for asthma includes the following topics:

  • Basic facts about asthma
  • Medications
  • Inhaler technique
  • Environmental control measures
  • Written asthma action plan developed with their PCP
  • Emphasis on the need for regular follow-up visits

Membership in the Ways to Wellness disease management program is voluntary.  This service is a benefit and there is no charge. If members choose to enroll, their covered services will remain the same.  When enrolled members get:

  • Support from a registered nurse
  • Education about your condition
  • Information on covered services        
  • Self-management techniques
  • Coordination of care

Education is provided over the phone or through the mail. Members don’t need to leave home to receive the services.
