Minnesota Birth and Death Records Application Forms are available on the Itasca County website.  Mail requests to Itasca County Recorders Office at 123 NE 4th St.  Grand Rapids, MN   55744-2600, along with a check or money order payable to Itasca County Recorder.  PLEASE NOTE - Forms must be notarized through mail.

Note: All the above fees are payable to the County Recorder via Check or Money Order. NO CREDIT CARDS.

Payments and necessary paper work required before documents are issued.

  • PASSPORTS BOOKS & PASSPORT CARDS (Adults 10 yrs / Minors 5 yrs)
  • Two passport photos required (must meet specifications/photos not taken at this acceptance facility)
  • Certified copy of Birth Certificate (* County or State with raised seal) HOSPITAL BIRTH CERT. NOT ACCEPTABLE
  • ITASCA COUNTY BIRTH CERTIFICATES Issued prior to 2001 are NOT ACCEPTABLE at this office
  • Valid Driver’s License or parent’s Valid Driver’s License if applicant is under age 16.  Child under 16 years old needs to be present along with both parents (need both parent’s Driver’s License)
  • Old Passport (if you had one before).  If you have an old passport & you are 16 or older you do not need to submit your certified Birth Certificate, only the old Passport.  If you have an old passport and are 15 or under you need to submit your Certified Birth Certificate and your old Passport.  
  • Do not sign application    
  • Have application completed, all white area’s except signature block, before returning to Recorder’s Office (DO NOT SIGN)
  • Everyone complete Parent Information (Even Adults)
  • Divorced & took back Maiden Name – need to submit Certified Court Papers
  • Recent ID issued within last year (including driver’s license renewal) must also submit copies of items such as: old driver’s license, other photo ID’s, hunting/fishing license, social security card, insurance card
